The French Tax Authorities (FTA) have recently published Forms 5000 and 5001 and their related notes under “Millésime 2018” on their website.

Clearstream Banking1 customers must provide the latest versions of Forms 5000 and 5001 completed according to the process described in our announcement A17150. Customers should also take into consideration the amended official notes on FTA’s website.
Background
The content of Forms 5000 and 5001 published on FTA’s website under “Millésime 2018” have not changed. The only noticeable difference compared to the ones under “Millésime 2017” is the designation of the ministry at the bottom of the page that changed from “Ministère des Finances et des Comptes publics” to “Ministère de l’Action et des Comptes publics”.
The main changes to this “Millésime 2018” are in the explanatory notes in which a few inaccuracies, noticed in the previous version and reported to the FTA, have been corrected.
In addition, a Chinese version of the forms is now available.
Impact on customers
When applying for tax treaty benefits on French-sourced dividends through the simplified procedure, quick refund (for securities deposited in CBF only) or standard refund, beneficiaries must always use the most up-to-date version of the forms as published by the FTA.
Customers are also reminded that Section III of the form was enhanced last year and a new sentence allows the beneficiaries to declare their taxability in their country of residence. Clearstream Banking requires Section III to be fully completed.
